Field trip permission, waiver, release and indemnity agreement

A parent or legal guardian must complete this agreement before a student takes part in an Intergalactic Adventures field trip.

IN CONSIDERATION of their student (the “Student”) being permitted to participate in any way in the field trip activity (the “Activity”), on site, the parent(s) or legal guardian(s) of the Student who sign this agreement hereby agree to the following terms and conditions:

1. Participation. I/We give permission for the Student to participate in the Intergalactic Adventures LLC (“IA”) field trip with the understanding that the Student is not mandated to attend. I/We understand and acknowledge that certain risks are inherent in this type of activity and I/we assume liability and responsibility for any such risks associated with participation.
2. Acknowledgement. I/We acknowledge, agree and represent that I understand the nature of IA activities and believe that the Student is qualified, in good health and in proper physical condition to participate.
3. Expectation. I/We understand the Student is expected to abide by all school, facility, location or organization regulations during the Activity, and agree to direct the Student to cooperate with the directions and instructions of supervisory personnel. IA defers to the medical release process of the school, facility, location or organization for medical care release and operation.
4. Hold harmless. I/We acknowledge that, as a condition of the Student's participation, I/we hold harmless and waive any and all claims against IA, its employees, agents, officers and volunteers, including but not limited to claims arising out of any ordinary negligence, or any loss or damage to personal property occurring during or by reason of the Student's participation.
5. Release from third-party liability. I/We understand that IA is not an agent of, and has no responsibility for, any third party including without limitation any sponsor, program or school that may provide services, equipment, location, training or activities associated with the Activity.
6. Indemnification. As a condition of the Student's participation, I/we indemnify IA for all claims resulting from the Student's participation, including but not limited to any injury, accident, illness, loss to self or of self, or any loss or damage to personal property.
7. Full understanding. I/We fully understand that activities INVOLVE RISKS AND DANGERS not limited to bodily injury. These risks may be caused by my Student's own actions or inactions, the actions or inactions of others participating, the conditions in which the Activity takes place, or the negligence of the releasees named above. There may be other risks and social and economic losses either not known to me or not readily foreseeable at this time, and I fully accept and assume all such risks and all responsibility for costs and damages I incur as a result of my Student's participation.
8. Voluntary agreement. The Student and the parent(s)/guardian(s) acknowledge that they have read this agreement and are aware of the legal consequences of signing this binding document. Signing this agreement indicates that I have read and freely agreed to it, and I further certify that I am legally competent to do so.
